Intake Paperwork

Our initial paperwork provides us with important information about you and your condition. We offer these forms online so you can complete it in the convenience of your own home. If you are unable to complete your paperwork ahead of time, please arrive at least 15-30 minutes prior early, to ensure you get the full amount of time with the doctor on your first visit.

Consultation, History and Examination

An initial appointment generally lasts 45-90 minutes. After checking in and obtaining vitals, you will have a consultation with the doctor to discuss your injury or health-related problems, concerns, as well as potential treatment options. In order to accurately determine your problem, the doctor will ask you various questions related to your condition. A number of specialized orthopedic, neurological, and physical tests will be performed to determine which tissues are affected and to what extent.

Diagnostic Studies

Your specific condition may require us to refer you for x-rays or advanced imaging to either rule out more serious conditions or assist us in developing the most effective treatment plan for you.

Treatment

Once finished with the examination and all appropriate studies, patients will generally be provided with their first treatment either on their first or second visit, depending on the results of the testing. This may include spinal manipulation, soft-tissue treatment, and therapeutic modalities, such as ultrasound and electrical muscle stimulation. An initial treatment plan will also be discussed at this time.

Home Instructions

Prior to leaving, patients will be given instructions on certain activities or procedures to be followed at home or work. This may include ice/heat application, home stretches and exercises, as well as the avoidance of certain activities.

Schedule Your Next Appointment(s)

We will schedule a convenient time for your follow-up visit. Generally speaking, patients are seen within 2-3 days of their initial visit and are provided with a complete review of diagnostic findings as received. Each initial treatment plan varies by the patient and their condition, but most patients can expect to attend at a frequency of twice a week, for 2-4 weeks.
